To obtain evidence of coverage for a special event or to add a party as a certificate holder, please complete the form below and submit your request.  Unless we need additional information, we'll email the certificate to you within one business day.

If this is an emergency, please call 911.

 If you have been in an accident or need to file a claim, please choose one of these options:

  1. You may contact your insurance company directly by phone, by email or website.  Specific information, including your policy number, is provided on the Contact List included in your ChurchInsure “Welcome” email.

Need to make Change to Your Policy?

Please contact us if you have made an important change at your church, such as:

  • Acquiring or building new facilities

  • Adding or subtracting staff positions

  • Adding or subtracting vehicles

  • Launching a new ministry, such as a school, camp or food bank
